Liability Waiver

This Liability Waiver and Release Agreement ("Waiver") is entered into by and between you ("Participant") and Puff & Go ("Company") regarding the use of hookah delivery services and equipment provided by the Company.

I acknowledge and understand that:

• The use of hookah and tobacco products involves inherent risks to health and safety
• Hookah smoking may cause serious health problems including but not limited to respiratory issues, cardiovascular disease, and cancer
• The handling of hookah equipment involves risks of burns, cuts, and other injuries
• Hot coals and heating elements present burn hazards
• There are risks associated with the consumption of tobacco and related products
• I am voluntarily participating in these activities with full knowledge of the risks involved

I hereby acknowledge that:

• I am in good health and have no medical conditions that would prevent me from safely using hookah products
• I am not pregnant, breastfeeding, or have respiratory conditions that could be aggravated by smoke exposure
• I understand that hookah smoking is not a safe alternative to cigarette smoking
• I have been advised to consult with a healthcare provider if I have concerns about tobacco use
• I am solely responsible for monitoring my own health and well-being during and after using Puff & Go services

In consideration of being permitted to use Puff & Go services, I hereby release, waive, discharge, and covenant not to sue Puff & Go, its owners, employees, agents, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to:

• Any loss, damage, or injury, including death, that may be sustained by me
• Any loss, damage, or injury to property owned by me
• The use of hookah equipment and tobacco products provided by Puff & Go
• Any acts or omissions by Puff & Go or its representatives
• Transportation and delivery of equipment
• Setup, operation, or maintenance of hookah equipment

This release applies to all claims whether arising from the ordinary negligence of Puff & Go or otherwise, to the fullest extent permitted by law.

I agree to indemnify and hold harmless Puff & Go from any loss, liability, damage, or cost (including attorney fees) that may arise from:

• My use of Puff & Go services and equipment
• My negligent or intentional acts or omissions
• My violation of any laws or regulations
• Any claims brought by third parties related to my use of the services
• Damage to property caused by my actions or negligence

I acknowledge and agree that:

• I am responsible for the proper use and care of all equipment provided by Puff & Go
• I will follow all safety instructions and guidelines provided
• I will not allow minors (under 21) to use or access the equipment
• I will immediately report any damage or malfunction of equipment
• I am financially responsible for any damage to equipment caused by misuse or negligence
• I will ensure equipment is returned in the same condition as received

• I certify that I am at least 21 years of age
• I have read this entire waiver and understand its contents
• I understand that this waiver is binding on my heirs, executors, and assigns
• I understand that this waiver cannot be modified except in writing signed by Puff & Go
• If any portion of this waiver is deemed invalid, the remainder shall remain in full force
• This waiver shall be governed by the laws of the State of Florida

If you do not agree to these terms, please do not use Puff & Go services.
